*WE REQUIRE A BORDETELLA VACCINE BE GIVEN WITHIN 6 MONTHS PRIOR TO BOARDING.
ALL VACCINES MUST HAVE BEEN GIVEN BY A LICENSED VETERINARIAN AND A WRITTEN NOTICE

*Vaccines required for Dogs to stay: Distemper / Parvo combo, Rabies, Bordetella every 6 months
(vaccine titers may be accepted).
*Vaccines required for Cats to stay: Distemper combo (Fvrcp), Rabies.
*Mealtime Instructions: We feed Diamond Lamb & Rice and/or Purina Pro Plan in our resort AM and PM feedings.
You may provide your own food. Please bring in serving size baggies/containers.

3. We will also apply a Flea/Tick Prevention upon admission to the boarding facility at a cost of $15.00.

4. All charges incurred by the owner shall be paid 50% at drop-off and balance upon pick-up of pet.
5. If your pet requires Veterinarian care while at Lake Whitney Pet Resort, and you or your
Representative cannot be reached, we have sole discretion to engage in any medical decisions
that need to be made concerning the health and safety of your pet and other’s at the Resort under our primary medical provider,
Squaw Valley Veterinary and/or Dr. Mark Jackson Meridian Veterinary Clinic. Maximum Dollar amount allowed of $________
for the emergency care of above animal.
6. Reservations are highly recommended. Cancellations with less than 24 hours notice will be
charged.
